APPLICATION CHECKLIST

This application checklist is for the upcoming school year. 
The 2025-2026 application will not be available until February 1, 2025.  Please do not complete the application prior to February 1st if you are a new family as you will have to complete a new one for the upcoming school year.  We will not transfer the information from a previous application. 

One the application is available, please note, that there is an application for PK3-Kinder and an application for 

1st -8th grades.  Please complete the correct application.

• Active Catholic Family Verification
• AfterCare Form 
• Catholic School Health Report (Physical)
• Medication Form
• Special Testing and Teacher Recommendation
• Student Emergency Care Form
