APPLICATION CHECKLIST

This application checklist is for the 2026-2027 school year. 
The 2026-2027 application will not be available until February 1, 2026 for new families.  Please do not complete the application prior to February 1st if you are a new family as you will have to complete a new one for the upcoming school year.  We will not transfer the information from a previous application. 

Please complete the correct application.

3) Upload all documents necessary to complete application:                                                    

• Immunization Form
• AfterCare Form 
• Catholic School Health Report (Physical)
• Medication Form
• Special Testing and Teacher Recommendation
• Student Emergency Care Form
